How is a rhinoplasty performed?

There are two basic techniques used in rhinoplasty procedures: open and closed. Either may be performed under IV sedation or under general anesthesia, and both are usually outpatient operations. During a closed rhinoplasty, Dr. Cohen works through an incision made inside the nostrils, while in an open rhinoplasty he makes the incision across the tissue between the nostrils (known as the columella).

Whichever method is appropriate in your case, Dr. Cohen will gently lift the soft tissues covering the nose and sculpt the bone and cartilage to the desired shape. If necessary, he will use additional cartilage removed from the septum (the bone and cartilage within the nose that separates the nostrils) to augment other nose tissue. If you have a deviated septum, meaning your septum is crooked and interfering with optimum breathing, during your rhinoplasty Dr. Cohen will adjust the septum (septoplasty) and other inner structures to remedy the problem. Once the difficult part of the procedure is finished, Dr. Cohen will carefully replace the lifted tissue and stitch it closed.

Septoplasty May Be Performed with or without a Rhinoplasty

While the septoplasty may be performed during the rhinoplasty, as described above, in some cases only a septoplasty is necessary. The nasal septum has three functions: to support the nose, to support the mucous membranes, and to help regulate nasal airflow. While a septoplasty may be indicated primarily as a way to address medical concerns, it may also be necessary in conjunction with a rhinoplasty in order to ensure that the reshaping of the nose does not reduce the patient’s ability to breathe.

Although the nasal correction will be immediately visible after the procedure, you should be prepared for some puffiness, swelling, bleeding and bruising. Most patients feel like themselves within a few days and return to work in about a week. The results of rhinoplasty become gradually apparent as the days pass after surgery and swelling recedes. Minor swelling may be present for 6 months. It is typically more noticeable in the morning and fades during the day. Contact lenses can be worn immediately, but glasses may need to be taped to your forehead or propped on your cheeks for up to six weeks.
